3. Financial Agreement & Insurance

By seeking treatment at MACH Therapy, you agree to the following financial terms:

  • Insurance Billing: We will bill your insurance provider as a courtesy. You must provide a current insurance card and ID.

  • Authorization: You authorize direct payment of insurance benefits to MACH Therapy.

  • Responsibility: You are responsible for all charges not covered by insurance, including deductibles, co-pays, and non-covered services. Co-pays are due at the time of service.

  • Referrals/Scripts: If you elect "Direct Access" without a physician's referral, you accept financial responsibility if your insurance denies payment due to lack of medical necessity.

  • Past Due Accounts: Accounts referred to collections will be subject to interest (18% per annum), collection fees (up to 50%), and attorney fees.

  • Cancellation Policy: Failure to cancel an appointment at least 24 hours in advance may result in a No Show/No Call fee of up to $100. This fee is not covered by insurance.

4. Communication Consent

By providing your contact information, you expressly authorize MACH Therapy and its service providers to contact you regarding your account, appointments, or services.

  • Methods: We may contact you via telephone (including wireless numbers), text messages, or email.

  • Automated Systems: This may include pre-recorded voice messages or automatic dialing systems.

  • Opt-Out: You may opt out of text or email communications at any time by notifying front desk staff, using the "unsubscribe" link in emails, or replying "STOP" to text messages.

5. Non-Discrimination Statement
